This blog is related to my autobiography DMD Life art and me plus there will be non related posts. I have the disease Duchenne Muscular Dystrophy and that has left me in a near paralyzed state, I wrote this book in 10 months using one finger clicking one mouse button on one on screen keyboard! To cough assist or not cough assist

It's increasingly common to see cough assist machines suggested for use with people who have DMD. For some it's an an absolutely fantastic invention but for others it might not be the ideal solution. The machines themselves work by forcing a large amount of air into the lungs and then quickly sucking it back out in an attempt to dislodge any secretions through the mouth. I've had a mini tracheotomy for 11 years and when I first had it put in there was no cough assist devices anywhere to be seen. I am very comfortable and used to suctioning through my mini tracheotomy and it gets my secretions straight out. A couple of years ago I was given the opportunity to try a cough assist machine out to see if it helped. I had a terrible week with a great  deal of pain in my chest from using the machine twice a day with no noticeable movement of secretions when I needed them removed. Suction through my mini tracheotomy was by far better for me so I decided it wasn't right for me and sent it back to benefit someone who could find relief in cough assist machinery. My lung team at the hospital often bring up its usage to me even though they know I'm not going to use it. It helps to point things out to healthcare specialists because you need to set clear boundaries. Don't let my experience put you off, I have this advice if you're just starting to have coughing troubles (i.e. you've lost the ability to cough) then definitely do try it out first. If like me you're a long term suction user through a tracheotomy then do try it but don't get your hopes up too high.

Foreword

I’m Ian Griffiths from South Wales. This book is a story of my life so far up to the age of twenty five years. I live with and suffer from the ill effects of DMD which stands for Duchenne Muscular Dystrophy. It is a severe muscle wasting disease and a life limiting terminal illness. It won’t kill you in six months in the traditional sense of ‘terminal’, but it’s far crueller than that, it steals every muscle in your body first and then kills you, anywhere up to the age of thirty. There have been cases of men living past that into their forties and fifties but only with drastic interventions such as ventilators and tracheotomies, more on this can be found by reading on.
I hope to cover a few things in this book, from a history of my childhood years to a more detailed history from sixteen years onwards and finally onto my current problems and triumphs. At times things I write may make you smile or may make you pause and think about the seriousness of life with this devastating disease. I really hope there will be a cure but currently for us supposedly ‘older’ guys with DMD (over twenty one), there seems very little hope. If I don’t see a cure in my lifetime, I hope my campaigning helps in some way bring it about for future generations, so another child won’t have to see their body wither and die before their time

The daily uncertainty

With DMD you never know what another day will bring. I know in life everybody has this same dilemma but most people wouldn't think that tomorrow they couldn't do something they did yesterday like writing with a pen. I have no idea if I can move my computer mouse the next day or whether a new pain would come along. (Coincidentally it's been many years since I held a pen!) Every month or so when I least expect it my fingernails get that bit too long. To most that would be no problem but my blog post describing my fingernail cutting exploits certainly shows how difficult it can be for me! Increasingly it's getting harder to use my mouse when my nails are only a tiny bit longer, it's amazing how difficult it becomes just to click the button. I never really know what could next fail and that's where being positive and hopeful comes into its own. DMD can certainly make you think and that's something it will never take away from me, the power of thought.

Puree pioneer

Ever since 2007 I've been eating a fully pureed diet and really enjoying food again. Before '07 I was struggling to shove potato fritters and breaded fish past mouth into my stomach. Feeding time was not pleasurable at all because I thought the next mouthful would lead to a choking incident. I had dropped weight considerably too and when I discovered I was 88 pounds I knew things had to change. Seemingly by coincidence I was given pureed food to try and finally I enjoyed a meal!
There are a few tiny inconveniences however as the variety is quite limited and you miss eating different textures. You can deal with that by remembering food is you're energy and it's better to eat than not. Although I'm slowly increasing the amounts of variety like this morning I tried some macaroni cheese from a tin and poured it over some liquidized scrambled eggs. I really enjoyed the creamy cheese and pasta and was extremely happy that it blended perfectly. I'm becoming increasingly adventurous with different kinds of recipes and have started adding garlic and occasionally herbs to my meals. A great resource I use for some of my recipes is from Leicestershire NHS pureed food guide find it here. So go on if you're struggling with solid food and have choked then why not try a pureed diet!

Mysterious illness

Over the last week I've had a strange chest problem where my lungs felt really terrible but I had no cold or flu symptoms. Obviously it appears to be a chest infection but how I ask myself. Well either something went into my chest via my mini tracheotomy or it was something else. I remember being told by an intensive care doctor that there was an increased risk of infection with a tracheotomy. While that's true we thought the benefits outweighed the risks and overall that has emphatically been true. Read more about mini tracheotomy tubes in my book. There is one other way I could have developed this illness that being aspiration. Basically that means swallowing food into the trachea and that usually leads to infections. Those with DMD may develop this if they have particularly weak swallowing muscles. Not every person living with DMD will weaken to the same extent in the same areas.  In any case I'm not sure what happened but I'm recovering now and that's the main thing.

Hoist hoorah

Finally I have my new hoist! We've had some teething problems with it, but we have a workaround and that has really improved the situation. It is so good to have that little bit of freedom back in my life. Having a bath was a great treat after weeks without even that basic thing! I had a similar feeling when my new lift was installed back in 2006, it was a mixture of relief and gratitude to those who helped me. There are always good feelings associated with DMD as well as the often negative you have just got to find them and cherish them. Like they say you can't appreciate the positive without experiencing the negative. Duchenne's certainly taught me that.

How do girls get DMD?

I mention quite a lot now that girls get DMD but how you might ask? Firstly you need to know how girls become carriers. The genetic problem (lack of dystrophin) occurs in the X chromosome. So if the genetic problem  happens to a boy because his genetic make up is XY he will always get DMD. A girl however becomes a carrier because one of her X chromosomes has the fault (their genetic make up is XX), but the remaining X takes over with it's code for dystrophin intact. Carriers can become severely manifesting carriers i.e. displaying some of the same characteristics of full blown DMD but have some sort of functional dystrophin.

Now the main point, (thanks to my friend Julie Gilmore for mentioning the term skewed X inactivation) how girls get DMD. I could be completely wrong about this but my limited understanding of skewed X inactivation is thus, instead of the other X chromosome taking over as per a carrier for some reason the other X does not work.  Hence the inactivation of the code for dystrophin. This may not be the only way girls get DMD but I hope this sheds some light on it. So girls in rare cases do get DMD!

Never Ending

My mum works incredibly hard every night. After an hour clearing my chest she has seemingly a never ending list of jobs to do. She truly is amazing! She treats my hands, feet and ears which need cleaning and some small medical treatments. Then she cleans my teeth and puts me in the lift. After arriving in my bedroom on the first floor she puts me into bed, puts my ventilator on and sometimes charges my wheelchair. THEN she can finally go to sleep. On a good night we might end up sleeping by 3AM on a bad night 4AM. I need to try harder to go to bed but it's difficult for both mum and me.

Unhealthy Tendencies

I used to drink gallons of fizzy drinks and eat all kinds of junk. I suppose I got into the destructive thinking of "what's the point in taking care of myself when I could die soon anyway". By living like that it was actually making me sicker, this would be true for everyone in that destructive pattern. So I realized that I needed to eat better since I had a reawakening so to speak.

Out goes the fizzy drinks and in goes the water, I can not eat junk now so I'm naturally protected from that. I eat meats, some fish, vegetables and fruit thanks to the pureed food my mum makes. I take vitamins and minerals too. It's definitely calmed my heart being off the caffeine and my skin and weight are all improved. If you eat well it also helps your mental state and I'm happier now. You may have a life threatening illness but there's no need to speed up the destruction in your body!

It was so liberating to go out to visit my friend yesterday. We drove through amazing breathtaking picturesque scenery. I really enjoyed that and loved the different colours of the trees. It's great not to think on DMD but to appreciate God's beauty. I even got some photo's in from mom (only a tiny reminder of DMD), great art inspiration. Just what I needed plus it was awesome to see my friends.

Foreword

I’m Ian Griffiths from South Wales. This book is a story of my life so far up to the age of twenty five years. I live with and suffer from the ill effects of DMD which stands for Duchenne Muscular Dystrophy. It is a severe muscle wasting disease and a life limiting terminal illness. It won’t kill you in six months in the traditional sense of ‘terminal’, but it’s far crueller than that, it steals every muscle in your body first and then kills you, anywhere up to the age of thirty. There have been cases of men living past that into their forties and fifties but only with drastic interventions such as ventilators and tracheotomies, more on this can be found by reading on.
I hope to cover a few things in this book, from a history of my childhood years to a more detailed history from sixteen years onwards and finally onto my current problems and triumphs. At times things I write may make you smile or may make you pause and think about the seriousness of life with this devastating disease. I really hope there will be a cure but currently for us supposedly ‘older’ guys with DMD (over twenty one), there seems very little hope. If I don’t see a cure in my lifetime, I hope my campaigning helps in some way bring it about for future generations, so another child won’t have to see their body wither and die before their time.

Hoist memories

Now that my hoists are both being replaced I started thinking about all I'd been through with them. I first remember using them when I was ten, when everything was new and it came with a blue sling that had metal attaching hooks. I remember falling out of an inferior sling thankfully not hurting myself! I've had thousands of baths and bed time routines through happy and sad times.

Extract;
.....hooks the grey sling surrounding my body onto the metal crossbar hanging from the hoist and I’m raised up into the air. My arms are carefully positioned on my legs so that my shoulders won’t hurt. Next I’m moved over my wheelchair by the touch of the button and carefully lowered in......

Hoist hassles

My dependable bathroom hoist has broken down after 20 years! It probably could be fixed but the hoist engineers aren't familiar with repairing it because of it's age. It amazes me how pervasive the throw away culture is now, if it's a little old or needs repairing instead of being innovative people would rather throw things away. Fair enough for something over 80 years but 20 years is nothing.

.... but for the dependent people like me we need our wheelchairs and ventilators charged and reliable power for suction machines, hoists, beds, mattresses and food blenders....

Too many alarms

The worst time for a power cut has to be 4 AM. I had my mask on and everything going black with no air on is certainly worrying. Eventually I had my mask removed and power was restored quickly phew! If the power goes off everything alarms, my air mattress, my special telephone and my ventilator all whine away. Once power returned everything went quiet but there was one alarm still wailing.

...Suddenly and unexpectedly I had to breathe for my self, quickly my heart started thumping [thankfully] the Nippy wailed out its alarm and I was quickly
reunited with the cool pressurized air that I was .... accustomed too....

Back to this mysterious wailing! We couldn't figure it out, convinced it was the bed we tried all we could to stop it. Eventually we tried switching off my ventilator, and blissful silence, you see we thought it was already off! That was really funny even at 4:30AM!
